Terms for All Membership Plans
- Month-to-month billing. No long-term contract.
- 30-day cancellation notice required in writing (1 billing cycle).
- Unredeemed benefits expire after 90 days.
- Slim Shot and NAD+ packages – as prescribed by licensed medical doctors on staff.
- Packages must be used within 90 days or any remaining injections will expire.
- Benefits may be shared with 1 designated person per contract terms.
- Medical treatments included in membership plans require complementary medical clearance for approval prior to use.
